- You will get IRS Form 1095-A (Health Insurance Marketplace Statement).
- Form 1095-A includes information you need to file your federal taxes.
How will I get my Form 1095-A?
- If you signed up for mailed notices, your form will be mailed to you.
- If you chose electronic notices only, you can download your form from your account.
- If you had coverage in 2026 or before, you would get a Form 1095-A for those years from HealthCare.gov. The form should be in your electronic inbox or available upon request by calling their customer service line: 800-318-2596.
- For coverage in 2027 and beyond, Oregon will provide your Form 1095-A form using the communication preference you chose in your application. You can also find this form in your secure online inbox.
