Reporting changes

Report any changes in your household, such as births, deaths, or changes in income. It ensures you are getting the right coverage and financial help.

If you or your family is covered on a plan purchase through the Marketplace, go to HealthCare.gov to make your updates.

If any of the information you provided on your Oregon Health Plan application changes, you must report it within 10 days of the change.

  1. Use your ONE online account to report changes online.
  2. Visit any Oregon Department of Human Services office in Oregon.
  3. Contact a local community partner.
  4. Call the ONE Customer Service Center weekdays at 800-699-9075.
  5. Fax to 503-378-5628 (Salem).
  6. Mail to ONE Customer Service, PO Box 14015, Salem OR 97309
