The COFA Premium Assistance Program

Health coverage for COFA citizens in OregonCOFA citizens are now eligible for the Oregon Health Plan

  • The COFA Premium Assistance Program closed Dec. 31, 2021. We will not pay premiums for 2022 plans. You can apply for Medicaid anytime during the year.
  • Visit ONE.Oregon.gov to apply for the Oregon Health Plan (OHP). 
    • If you are within the income limits below, you may qualify for Oregon Health Plan benefits:

  • If you are over income for the Oregon Health Plan, you can apply for financial assistance and may be able to enroll in a health plan through the Marketplace at HealthCare.gov.
  • If you paid out-of-pocket medical expenses in 2021 while enrolled in the COFA Premium Assistance Program, you have until April 30, 2022, to submit a claim for reimbursement.

COFA reimbursements
“Where’s my COFA reimbursement?” is updated once every 72 hours.

Please enter the last four digits of your Social Security number, your last name, and the refund amount as shown on your COFA Premium Assistance Program reimbursement claim form (#440-5133)  2020 | 2021.

When to check status of your reimbursement:
  • One week after faxing your claim form or
  • Two weeks after mailing your claim form

 Where's my COFA reimbursement?

Payment method change:

If you want to change how you receive your reimbursement refund from U.S. Bank Relia Card to check, we will mail an additional form to you needed for that change. Remember to sign and return the form so we can get your refund paid.
